    • The cover sheet should:
      1. Identify the business and the document
      2. Identify the location and telephone numbers of the business or where the principals can be reached
      3. Identify the person who wrote the business plan

    STATEMENT OF PURPOSE

    Who is asking for money? What is the business structure? (i.e., sole proprietorship, partnership, corporation, Subs corporation) How much money? What is the money needed for?

    How will the funds benefit the business? Why does the loan or investment make economic sense? How will funds be repaid?
